1. While applying for a Broadband Home Connection, Subscriber has to attach below copies of the following documents:

Proof of address (attach one of the following): Applicant’s ration card, certificate from Employer of reputed companies on letter head, water /telephone /electricity bill/statement of running bank account/Income Tax Assessment Order /Election Commission ID card. (NOTE: If any applicant submits only ration card as proof of address, it should be accompanied by one more proof of address out of the above categories).

• ID Proof (attach one of the following) Voter Id, Pan Card or Aadhar Card
• Passport size – 1 Photo
• All documents with KYC should be signed by Customer and same need to be attested by LCO

2. While applying for a Broadband SME Connection, Subscriber has to attach below copies of the following documents:

• Proof of address and ID: Company Registration Certificate (Corporation), Rental / Lease /Sale deed, Company Pan Card
• Passport size – 1 Photo (c) All documents with KYC should be signed by Customer and same need to be attested by LCO

3. While applying for a LCO Connection, Partner has to attach below copies of the following documents:

Proof of address (attach one of the following): Applicant’s ration card, certificate from Employer of reputed companies on letter head, water /telephone /electricity bill/statement of running bank account/Income Tax Assessment Order /Election Commission ID card. (NOTE: If any applicant submits only ration card as proof of address, it should be accompanied by one more proof of address out of the above categories).

• ID Proof – Pan Card
• Passport size – 1 Photo and (d) Post office certificate
